Applicant Declaration, Consent, and Indemnity

I confirm that all information provided in this application is true, complete, and accurate to the best of my knowledge. I understand that any false, misleading, or incomplete information may result in disqualification from the application process or withdrawal from the program.

I confirm that all materials submitted by me, including videos, written documents, designs, sketches, concepts, images, and other supporting files, are my original work or materials I am legally entitled to use and submit. I accept full responsibility for any copyright, ownership, plagiarism, privacy, or content-related claims arising from my submission.

I hereby release and hold harmless the Centre for Creative Arts Education, its trustees, management, staff, facilitators, affiliates, and partners from liability arising from the review, receipt, storage, handling, or assessment of my submitted materials, except where such liability cannot lawfully be excluded.

I grant the Centre for Creative Arts Education permission to receive, review, internally reproduce, store, and use my submitted materials strictly for application assessment, selection, training review, documentation, and program administration.

I understand that any promotional, public, advertising, publication, or broadcast use of my submitted materials beyond internal program-related purposes shall require separate consent where applicable.

I acknowledge that participation in a creative arts program may involve rehearsals, performances, collaboration, movement, travel related to approved activities, training exercises, critique sessions, and other educational or professional demands. I understand that participation may involve personal, physical, emotional, reputational, or professional risks, and I voluntarily accept such risks.

I agree to abide by the rules, code of conduct, safeguarding standards, and guidelines of the Centre for Creative Arts Education, and I understand that violation of these rules may result in suspension, disqualification, or removal from the program.

If I am under 18 years old, I confirm that this application is being submitted with the knowledge and consent of my parent or legal guardian and that the required consent documentation has been provided.
